HF-OCT can be effectively used with saline-only flushes, offering an important alternative for patients at higher risk of contrast-induced complications.
In this case, a pre-PCI assessment of the right coronary artery (RCA) was performed using saline only, demonstrating that HF-OCT can produce high-quality images even without contrast. The MLA was measured at 2.0 mm², and PCI was successfully guided based on these imaging results.
This approach is particularly valuable for patients at higher risk of contrast-related complications, such as those with renal impairment. When contrast is used with HF-OCT, clinical evidence demonstrates that significantly lower volumes are required, typically less than 5 mL per run compared with conventional OCT, while maintaining image quality and accurate lesion assessment.¹ The small 1.8 F catheter and fast-pullback imaging enable efficient blood clearance, reducing the need for contrast.¹
